Role overview

GreenWell Health is hiring a caring, organized Scheduling & Intake Coordinator to support patients, families, referral partners, and internal team members. This position focuses on delivering high-quality service, managing incoming referrals, answering service-related questions, and helping build strong relationships with providers and other referral sources.

What you will do

  • Accept and process referrals from discharge planners, physician offices, and facility personnel, using electronic messages, fax, phone calls, and Care Coordinator notifications to capture key details accurately in the EMR system.
  • Respond to questions from prospective patients and their families, collect essential intake details, explain how GreenWell Health can help, and help arrange a visit that begins the admission process in the EMR.
  • Work with Admissions staff to make sure the right team member makes the first contact, based on the patient’s condition and family circumstances.
  • Keep communication flowing with referral partners, attending physicians, coworkers, and outside agencies to support the start of hospice services.
  • Coordinate information across the sign-on, consent, and admission assessment stages, track referral and admissions data, follow up on open referrals, and update patient status when needed.
  • Develop and maintain professional relationships with referral sources, commercial insurers, and third-party insurers through prompt written and verbal communication.
  • Protect patient privacy and follow HIPAA requirements when handling confidential health information.
  • Take part in onboarding for new Care Coordinators, cross-training, continuing education, committees, special projects, and other assigned responsibilities.

Experience and education

Applicants should have a high school diploma; an associate or bachelor’s degree is preferred. The role calls for at least 2 years of office experience, including preferably 1 year in healthcare and ideally in a hospice agency office.
