Care Navigator

Role Overview

Dallas Medical Physician Group, part of Dallas Medical Center, is looking for a Care Navigator to join a mission-driven team focused on quality, compassion, and community impact. This full-time, day-shift position in Dallas, Texas involves helping patients move smoothly through care by removing obstacles, improving access, and strengthening communication among providers, patients, caregivers, and community partners.

The role also includes monitoring admissions and readmissions, documenting how patients are supported and what outcomes follow, and contributing to bundled payment and comprehensive joint replacement initiatives. The Care Navigator will work closely with stakeholders to increase awareness of hospital support programs, services, and community resources, while also helping identify patient and family needs and supporting ongoing program improvement.

Key Responsibilities

• Identify and address barriers that prevent patients from accessing care.
• Support patients with access-related concerns and coordination needs.
• Build and maintain relationships with healthcare providers and care teams.
• Ensure clear and effective communication among providers, patients, and caregivers.
• Coordinate services across the organization, outside facilities, and community resources.
• Monitor admissions and readmissions, including patient support efforts and resulting outcomes.
• Assist with bundle payment processes and comprehensive joint replacement programs.
• Partner with stakeholders to increase awareness of hospital support programs, services, and community needs.
• Gather and respond to concerns expressed by patients and families.
• Contribute to continuous review and improvement of the navigation program.

Education, Experience, and Skills

This position is best suited to candidates with experience in medical environments, strong coordination skills, and the ability to work with diverse groups of people. A bachelor’s degree is preferred.

Applicants should have at least two years of experience in a clinical or medical setting, working directly with medical staff and patients. Experience in post-acute care is preferred. Candidates should also understand community resources and have experience with community outreach.

The role requires a solid grasp of healthcare systems, medical terminology, and HIPAA rules, along with strong communication, interpersonal, public speaking, organizational, advocacy, and problem-solving abilities. Comfort with Microsoft Office and database tools is preferred. Bilingual ability is also preferred. The person in this role should be able to manage priorities, juggle multiple tasks, and work effectively both independently and as part of a team.
