About the Organisation
MINDS has supported persons with intellectual disabilities and their families since 1962. Built on the belief that every person has strengths and potential, the organisation works to improve quality of life, promote inclusion, and expand opportunities for people with intellectual disabilities across Singapore.
As one of Singapore’s largest charities with more than 1,200 employees, MINDS delivers a broad network of services across the lifespan of persons with intellectual disabilities. Its work spans schools, employment and training centres, home-based support, and community-focused programmes that address physical, psychological, environmental, and social needs.
Role Overview
The Community Care Associate supports persons with intellectual disabilities through skills training, intervention, and person-centred care. The role focuses on helping clients strengthen daily living abilities, participate more actively in the community, and improve independence and confidence. The associate also documents client progress, contributes to care planning, and works closely with caregivers, social workers, and community partners.
This is a full-time, onsite role based in Singapore. The working pattern is 5 days a week, Monday to Friday, from 9:00 am to 6:00 pm. The role requires travel across Singapore.
Key Responsibilities
- Support clients with personal care and help them build daily living and community participation skills.
- Assist allied health professionals in carrying out rehabilitation-related support for clients.
- Deliver training and activities in line with individual care and training plans.
- Share observations and inputs for the creation and review of care plans.
- Keep client records, progress notes, and condition updates accurate and current.
- Work with the multidisciplinary team to review client concerns and progress on a weekly basis.
- Assess functional abilities, contribute to personalised care plans, and take part in case discussions and evaluations.
- Develop and maintain suitable training activities and resources, ensuring care plans are implemented consistently and effectively.
- Prepare timely documentation, including progress records, care plan updates, and incident reports, according to organisational standards.
- Build positive working relationships with family caregivers, volunteers, befriending partners, and other community stakeholders.
- Explain client needs and intervention plans clearly to caregivers.
- Guide clients and caregivers on self-care techniques and the use of assistive devices.
- Share relevant updates on client progress with stakeholders.
- Respond appropriately to unusual or difficult client behaviours.
- Recommend risk-reduction measures with support from allied health professionals.
- Maintain a safe, hygienic, and orderly care setting by following health and safety advisories.
- Keep training materials clean, functional, and ready for use.
- Record incidents properly and use technology effectively in work and teaching activities.
- Run social, recreational, and educational programmes that encourage independence, wellness, and better quality of life.
- Collaborate with caregivers on training plans and identify opportunities to improve activities and programmes.
Requirements
- Minimum ITE qualification.
- Experience in training or educating persons with intellectual disabilities is an added advantage.
- Background in a multidisciplinary team environment is beneficial.
- Ability to take part in both on-site and off-site activities.
- Basic First Aid knowledge or certification is required.
- Prior exposure to home personal care or similar work is an advantage.

Work Arrangement
5-day work week, Monday to Friday.
Working hours: 9:00 am to 6:00 pm.
Travel within Singapore is required for the role.

The position is part of Community Based Support Services, which supports persons with intellectual disabilities and their caregivers through community-based programmes and services, including Me Too! Club, Future Care Planning Services, Caregivers Support Services, Special Student Care Centres, Disability Case Management Programme, and MINDS Clinic.
The role calls for a person-centred approach to improve independence, confidence, and quality of life for clients while strengthening their support network with help from social workers and community partners.
