Role overview

KeystoneCare Home Health and Hospice is looking for an Intake Coordinator to help manage the entry of patients into its care services. In this role, you will be the first contact for many patients and referral partners, explaining available services, reviewing incoming referrals, collecting accurate details, and routing cases to the right clinical team. Responsibilities

  • Handle referrals that come in through calls, emails, and referral networks.
  • Explain the agency’s services and care capabilities to patients and referral sources.
  • Review each referral to determine whether it can be accepted based on service area, clinical needs, and insurance coverage.
  • Record patient details accurately in the electronic medical record system.
  • Assign patients to clinical teams according to their needs and staff availability.
  • Help ensure care begins within required regulatory and agency timeframes.
  • Keep patients, caregivers, and referral partners informed about care start dates and any updates.
  • Adjust clinician schedules when planned or unexpected absences occur.
  • Support nurses so they can meet daily and weekly productivity targets.
  • Deliver high-quality service to internal teams and external contacts.
  • Protect patient confidentiality at all times.
  • Take on additional tasks assigned by management.

Requirements

  • At least 3 years of intake experience in a healthcare setting, ideally within home health or hospice.
  • Ability to juggle multiple priorities in a busy environment while communicating clearly.
  • LPN qualification is preferred.
  • Strong client-service approach and the ability to work well with healthcare staff, patients, and families.
  • Good data entry and scheduling abilities.
  • Self-motivated and comfortable working independently as well as with a team.
  • Experience using electronic documentation tools.
  • Proficiency with Microsoft Word, Excel, and Outlook.
