About the Role

CommWell Health is seeking a clinician to deliver primary care services in a rural North Carolina setting. What You Will Do

• Deliver hands-on outpatient primary care, including assessment, diagnosis, treatment planning, and charting in line with established protocols.
• Evaluate patients, prescribe appropriate medications, and arrange laboratory work or imaging when clinically indicated.
• Oversee chronic condition management, encourage patients to meet self-care goals, and track their progress over time.
• Work closely with multidisciplinary care teams, take part in case discussions, and support others as a mentor or preceptor.
• Stay involved in continuing education, quality improvement activities, and initiatives tied to the patient-centered health home model.

Required Qualifications

• Current BLS certification from either the American Red Cross or the American Heart Association.
• Active, full authorization to practice medicine issued by the NC Board of Nursing or the NC Board of Medical Examiners, along with a valid license.
• National certification as an MD, DO, FNP, or PA.

Preferred Background

• At least one year of professional experience.
• Prior experience serving rural or underserved communities.
• Exposure to team-based care models or PCMH/PCHH frameworks.
