About the Role
This position sits within a Mobile Nursing service aimed at improving access to health and social support for people and whānau across the Wairoa District. The role focuses on helping individuals maintain better health and wellbeing through practical support, healthy lifestyle promotion, and stronger links to services and community resources.
Role Purpose
The Community Support Worker - Health will work in partnership with Mobile Nursing Registered Nurses to help individuals and whānau achieve better health outcomes.
Key Responsibilities
- Assist individuals and whānau in lifting their health and wellbeing.
- Provide guidance and education that encourages healthier daily choices and lifestyles.
- Link clients with relevant health, social, and community-based services.
- Keep accurate client notes, assessments, and care plans up to date.
- Collaborate with nurses, whānau, and local partners to support positive outcomes.
What We Are Looking For
The ideal candidate will bring experience in supporting whānau, community groups, or health-focused environments, along with strong interpersonal and communication skills. A good understanding of tikanga Māori and a commitment to Te Tiriti o Waitangi are important, as is the ability to stay organised, manage time well, use computer systems confidently, and hold a full driver's licence.
Desirable Experience
It would be an advantage to have worked in health or community services before, together with fluency in Te Reo Māori and knowledge of tikanga Māori. Familiarity with Te Whare Tapa Whā and RBA is also desirable.
