Role Overview

The Homeless Pathway Coordinator ensures that no patient leaves a hospital without a safe place to go. This role involves supporting homeless patients by navigating the complex networks of health, housing, and support services. Working primarily within Newham, London, the Coordinator will regularly visit patients in hospital wards and A&E, listening attentively, identifying obstacles, and coordinating comprehensive discharge and support plans that prioritize patient preferences.

Key Responsibilities

• Manage individual caseloads by conducting assessments and creating clear discharge and support strategies.
• Refer patients to appropriate housing, healthcare, social care, mental health, and substance use resources.
• Advocate strongly to prevent patients from being discharged without sufficient accommodations or support.
• Collaborate with hospital teams, Newham Council, the Homeless Pathway Lead, and the wider health team to align on patient-centered care.
• Build linkages with community services, identify gaps in existing pathways, and contribute to improved hospital approaches for supporting homeless individuals.
• Engage in training, reflective practices, and shared learning to continuously improve expertise in inclusion health and psychologically informed care.

Candidate Profile

• Experience working with individuals facing homelessness, multiple disadvantages, or substantial barriers to service access.
• Ability to foster trust with people who may have negative experiences with support services, ensuring their perspectives and choices remain central.
• Competent in caseload management and converting complex issues into actionable plans.
• Effective communication and advocacy skills facilitating collaboration between healthcare, housing, and support providers.
• Calm and compassionate approach with clear understanding of safeguarding and risk management.
• Motivated to enhance knowledge through ongoing training and reflective practice.

Benefits

• Starting salary ranging from £34,127 to £36,947 annually (pro-rated).
• Salary increment after successfully completing a six-month probationary period.
• 37.5-hour workweek with flexible hours (core hours 10 am – 4 pm) for non-accommodation services.
• 25 days of annual leave, increasing up to 30 days plus paid bank holidays.
