Job Overview
As a Social Work Assistant at SingHealth Community Hospitals, you will collaborate with Medical Social Workers (MSWs) by performing home visits to gather collateral data and assess the patient's home and family environment. Your role includes aiding MSWs in interviewing and evaluating patients and their families to facilitate appropriate service referrals. Additionally, you will manage referral registrations received by the department, conduct intake interviews on behalf of MSWs, and carry out means-testing for patients applying for subsidies for community hospital services in accordance with Ministry of Health guidelines.
Eligibility and Requirements
- Diploma holders, ideally in Community Services or Human Services, with a minimum of 4 years relevant experience
- Alternatively, candidates holding GCE ‘O’ or ‘A’ level qualifications with at least 10 years of relevant experience
