Overview

The Community Transformation initiative represents Pennine Care NHS Foundation Trust's largest change programme, aligning with the Community Mental Health Framework and NHS Long Term Plan. It aims to establish a modern, integrated, place-based community mental health model focused on whole-person and whole-population health, closely connected to Primary Care Networks.

This opportunity is for an experienced project manager to assist the programme team in achieving these transformative goals by collaborating across clinical and operational teams within the Trust and broader Greater Manchester partners.

Key Responsibilities

  • Support the Programme Director, Senior Programme Manager, and Strategic Operational/Clinical Lead in driving change.
  • Provide expertise in project and improvement methodologies to aid the coordination, scoping, and delivery of programme work streams.
  • Ensure projects adhere to the Trust’s Quality Improvement Framework and are effectively monitored.
  • Assist in progressing community pathways to bring care closer to home, enhancing resilience and resourcefulness within community services to reduce hospital admissions and system costs.
  • Support specific projects such as developing Living Well models, enhancing Community Mental Health Teams, transitioning from traditional care coordination to Multi-Disciplinary Team key worker approaches, implementing DIALOG+ and Patient Reported Outcome Measures in place of Care Programme Approach, and fostering new adult social care integration models.
  • Provide operational support across five PCFT localities under the guidance of senior leadership, streamlining timelines, managing logistics, supporting governance, and monitoring KPIs aligned with programme objectives.
