Patient Mobilization Coordinator

Role Overview

The Patient Mobilization Coordinator plays a pivotal role in managing and streamlining patient acquisition and care processes within outreach locations and mobile clinics. This position involves overseeing patient journeys from initial identification through treatment completion while engaging community partnerships and managing logistics to ensure patient satisfaction and care efficiency.

Primary Responsibilities

• Lead coordination efforts for identifying, sourcing, and mobilizing patients across outreach sites and mobile healthcare services.
• Manage the complete patient care cycle, including steps prior to admission, during admission, treatment, discharge, and subsequent follow-up care.
• Track and facilitate patient transitions from referral sources through to the conclusion of treatment ensuring effective pipeline management.
• Drive community awareness initiatives and strengthen referral networks and partnerships to support patient mobilization.
• Organize and oversee patient transportation logistics in line with organizational programs and support community-driven initiatives.
• Establish and maintain communication systems that gather patient feedback and enhance overall patient experience.
• Support patient follow-up via home visits and proper documentation procedures.
• Carry out social work-related activities targeting patient mobilization, recruitment, and follow-up to reinforce program objectives.

Secondary Tasks

• Collaborate closely with Business Development, Programs, Clinical, and Booking teams to optimize patient scheduling and care pathways.
• Produce detailed reports on patient mobilization efforts, referral tracking, and conversion rates to inform operational strategies.
• Contribute actively to meeting hospital-wide patient growth goals.
