Role Overview

The Lumiere Foundation seeks a skilled NDIS Support Coordinator to assist ICCare participants by managing diverse levels of support coordination and transitions within their NDIS plans.

Key Responsibilities

  • Follow and uphold NDIS and Child Safe standards and regulations rigorously.
  • Coordinate participant supports effectively within allocated funding.
  • Collaborate with key stakeholders to align service delivery towards participants' goals.
  • Empower participants by facilitating choice and control to access various formal and informal community supports, emphasizing innovative service options.
  • Maintain precise and clear records, files, and reports adhering to guidelines, standards, and legislative requirements related to NDIS and Quality and Safeguards.

Qualifications and Experience

  • Possess tertiary qualifications in social sciences, health, or a related professional discipline, or a diploma in Disability Services studies.
  • Have a minimum of two years’ practical experience in Support Coordination.
  • Familiarity with NDIS Quality and Safeguards Commission practice standards.
  • Proficiency with Microsoft Office Suite (Outlook, Word, Excel) and experience utilizing CMS systems such as MYP, Lumary, or ShiftCare.
  • Arabic language skills are advantageous but not mandatory.
  • Strong verbal and written communication skills in English, including report writing capabilities.

  • Hold a current National Police Check.
  • Possess a valid First Aid Certificate.
