Role Overview
The Care Coordinator is responsible for collaborating with administrative staff, Care Managers, patients, and their families to facilitate healthcare objectives and maintain open communication channels. This role primarily supports non-clinical duties, providing additional assistance to clinicians, case managers, and members in relation to patient care.
Key Responsibilities
- Act as a resource and educational contact for patients, families, providers, and staff.
- Conduct telephonic follow-ups with members, including post-hospital discharge and chronic condition management.
- Carry out assigned Care Coordination tasks diligently.
- Create tasks for the assigned Registered Nurse Case Manager.
- Guide patients and families to suitable resources, including community services.
- Under the supervision of the RN Case Manager, assist members in achieving their healthcare goals through support, education, resource access, and arranging follow-up care.
- Ensure quality service by adhering to organizational standards.
- Assist with data entry, faxing, filing, and other clerical responsibilities.
- Manage inbound and outbound calls, obtaining information as requested by licensed staff.
Qualifications
- Minimum of one year experience in a healthcare-related position.
- High school diploma or GED required.
- Strong organizational and time management capabilities.
- Excellent verbal and written communication skills.
- Proficient in professional and efficient telephonic communication.
- Computer skills including Microsoft Office Suite (Excel, Word, Outlook, PowerPoint).
- Ability to multitask effectively in a dynamic environment.

Physical Requirements
