About Lifelong

Lifelong is a community-focused health organization dedicated to eliminating barriers to health through compassionate care. With a 40-year legacy, Lifelong addresses health disparities by providing comprehensive services including food and nutrition, healthcare navigation, aging and disability support, and housing resource connections.

Position Overview

The ADS CMP Supervisor oversees daily administrative functions within the program, managing assessment processing, case management support, technical aid, and coordinating with providers offering in-home services to elderly and disabled adults receiving Medicaid-funded personal care and related support. This role includes performing case management duties up to 8 hours weekly, striving to meet 20% of standard case manager productivity, alongside supervisory responsibilities constituting the majority of the workweek. The supervisor also aids in training new case managers, policy development, and ensures effective program functioning, reporting directly to the ADS Program Manager.

Responsibilities

  • Conduct client assessments annually to determine service needs and eligibility, develop care plans, authorize services and providers, and monitor plan implementation through home visits and calls.
  • Educate clients and support networks on care plans to prevent unnecessary institutionalization and guarantee culturally appropriate, high-quality service delivery.
  • Manage service terminations by notifying clients of changes, providing planned action notices, making referrals, and closing services as needed.
  • Maintain timely communications with coworkers, stakeholders, clients, funders, and management about client and program status.
  • Promote quality clinical services aligned with social work ethics and best practices.
  • Collaborate with other managers for cross-regional service coordination and offer clinical supervision and support using internal and external resources.
  • Participate in recruitment, onboarding, and training of new staff in coordination with Program Manager and Director.
  • Oversee administrative processes including tracking deadlines, managing caseloads, workflows, staff scheduling, and ensuring compliance with policies.
  • Foster positive team culture, mediate conflicts, handle client grievances, and provide ongoing staff feedback and guidance.
  • Assist with payroll processes, contract deliverables, quality assurance activities, data management systems, and administrative hearings.
  • Coordinate medical equipment ordering and ensure staff training on documentation and quality standards.

Qualifications

  • Minimum two years of relevant experience with aged or disabled populations and leadership exposure.
  • Proficient with various database systems and Microsoft Office suite.
  • Strong organizational skills to balance multiple projects and meet deadlines in fast-paced settings.
  • Ability to navigate complex federal and state regulations and produce detailed technical documentation.
  • Clinical healthcare background, familiarity with chronic health management, and experience with behavioral challenges.
  • Knowledgeable about Washington State Long Term Support Services and Medicaid Case Management.
  • Bachelor’s degree required, Master’s preferred, in relevant fields such as social services or public administration, or equivalent combination of education and experience.
  • Alternative minimum qualifications include combinations of social service experience, supervisory roles, bilingual skills, and completion of leadership training.

Work Environment and Conditions

  • Position requires physical activities including sitting, standing, walking, bending, reaching, and occasional lifting up to 20 pounds.
  • Work occurs in office, residential, community, and sometimes outdoor settings, occasionally outside of normal business hours.
  • Job requires passing criminal background checks and TB testing as mandated.

Benefits

  • Comprehensive medical, dental, and vision plans.
  • Generous paid time off including vacation, sick leave, and personal days.
  • 401(k) retirement plan with employer match.
  • Flexible spending accounts, life insurance options, long-term disability coverage.
  • Mass transit subsidies and 15 paid holidays annually.

Additional Information

Lifelong values diversity and inclusion, encouraging candidates from all backgrounds to apply even if not all qualifications are met. The employer supports equal opportunity, maintaining a non-discriminatory work environment. Job descriptions are not contracts but serve to guide hiring and employment practices.
