About the Role
The Education Continuity Coordinator (ECC) plays a pivotal role in guaranteeing ongoing educational access and rights for adolescent and young adult clients enrolled in Sandstone Care's Partial Hospitalization Program (PHP). Working in a setting regulated by behavioral health and state education authorities, this position acts as the main intermediary between the treatment program and external educational institutions, including home schools, Local Education Agencies (LEAs), and post-secondary education bodies within Colorado and Maryland. The ECC ensures students' academic progress, rights, and school affiliations are maintained seamlessly during treatment, and facilitates a clear and actionable transition plan upon clients’ return to their schools of record.
Key Duties
- Perform detailed educational screenings of new clients to assess needs.
- Analyze and apply knowledge of Individualized Education Plans (IEP) and Section 504 Plans to support continuous learning.
- Offer academic and vocational services such as assessments, goal development, and coordination with external educational or vocational providers, including families.
- Ensure prompt and accurate documentation of educational services provided.
- Support and motivate clients to complete homework assignments regularly.
- Build and maintain positive relationships with schools, families, referral sources, and community partners.
- Communicate effectively and persuasively in various settings.
- Exercise sound judgment and emotional stability when managing stressful situations or deadlines.
- Participate in Individualized Education Plan (IEP) meetings as needed.
- Organize and support daily structured study sessions.
- Collaborate with Site Directors to monitor clients’ expected program durations.
- Maintain detailed communication logs with schools for all active clients.
- Develop written Academic Transition Plans for clients prior to discharge.
- Engage in monthly quality assurance evaluations.
Qualifications
- Bachelor’s degree required; preference given to candidates with degrees in Education or Behavioral Health Sciences (e.g., clinical social work, psychology, marriage and family therapy).
- Possession or eligibility for a teaching license in the state of employment.
- Proficient understanding of the Individuals with Disabilities Education Act (IDEA).
- Experience mentoring adolescents and delivering academic, vocational, and life skills support.
- Minimum of three years’ experience working with clients aged 13 to 18 is ideal.
- Strong written and verbal communication capabilities.

Work Schedule and Location
This full-time role is based in outpatient treatment centers for teens and young adults, operates Monday through Friday with occasional evenings required, offering a hybrid work model across Colorado and Maryland.
