Position Overview
The Hospice Liaison will be responsible for delivering professional and thorough presentations of the Company’s hospice services to the healthcare community and potential hospice candidates within an assigned territory. The role aims to meet referral objectives and boost service revenue. As a resource, the liaison will also provide outreach and education to physicians, healthcare facility staff, clients, and caregivers about hospice care.
Key Responsibilities
- Achieve or surpass monthly targets for qualified referrals and admissions.
- Exhibit accountability for punctuality, effective use of work hours, and mostly independent work through excellent time and territory management.
- Prepare in advance for scheduled meetings and appointments.
- Promote a positive environment and enhance morale among staff, facilitating open communication across departments.
- Engage in organizational events to increase community and public awareness of Peoples Hospice and Palliative Care offerings.
- Collaborate with peers and executive leadership to establish pricing strategies that optimize profitability and market share while maintaining customer satisfaction.
- Track healthcare trends to recognize needs for new services and manage product development accordingly.
- Stay informed about public health trends and medical needs.
- Evaluate referral sources for potential patient and revenue value.
- Assess account potential and allocate efforts based on expected returns.
- Negotiate mutually beneficial contractual agreements with institutions.
- Identify primary customer needs and decision-making contacts to effectively close agreements.
- Conduct follow-ups to ensure execution of proposals.
- Evaluate patients’ medical conditions and family requirements, collaborating with clinical managers and physicians to confirm hospice eligibility.
- Coordinate prompt medical actions on admission day per physician directives.
- Report clinical information and issues in patient care meetings to ensure continuity and coordination with interdisciplinary groups, particularly informing assigned case managers and relevant disciplines.
- Keep detailed records, manage budget expenses responsibly, and submit timely reports.
- Pursue ongoing professional development via workshops and in-services.
- Maintain medical ethics consistent with company policies.
- Adhere to state and federal regulations governing Medicare-certified hospice operations.
- Project a professional demeanor and use proper telephone etiquette.
- Encourage a welcoming environment for visitors while safeguarding patient confidentiality.
- Participate in accreditation initiatives.
- Perform additional duties as needed.
Required Qualifications
- High school diploma mandatory; bachelor’s degree preferred.
- Experience in healthcare with significant exposure to sales and marketing roles.
- Prefer familiarity with hospice care practices both practically and theoretically.
- Strong interpersonal, coordination, team-building, and leadership abilities, especially in communicating with physicians and senior executives.
- Demonstrated knowledge of financial management principles.
- Proficient in Microsoft Word, Excel, and PowerPoint.
- Skilled in prioritizing tasks, managing time effectively, and making independent decisions as necessary.
