Job Overview
We are seeking a proactive Band 4 Therapy Assistant to join our Community Services team, focusing on triage and clinical support duties. Partnering closely with a multidisciplinary team, you will help ensure smooth patient pathways and deliver excellent care.
Key Duties
- Assist in triage activities across community services by supporting referral screening and prioritisation.
- Communicate with referrers to obtain further clinical details and clarify referral requirements.
- Process and allocate referrals promptly to maintain patient flow.
- Carry out delegated clinical tasks as directed by qualified therapists, including participation in patient assessments and rehabilitation plans.
- Monitor patient progress and escalate concerns when necessary to ensure patient safety.
- Maintain precise and thorough clinical documentation in line with Trust standards.
- Collaborate effectively with therapists and the wider multidisciplinary team to support patient care.
- Handle administrative tasks such as preparing correspondence and managing patient information systems.
- Promote access to community resources and liaise with other care providers as needed.
- Manage risks to patients and maintain respectful, professional relationships with patients, clients, and staff.
Skills and Experience
- Previous experience in healthcare or community service environments.
- Strong interpersonal and communication skills, especially in liaising with clinical referrers and team members.
- Ability to prioritise tasks effectively and work independently as well as part of a team.
- Knowledge of patient-centred care principles and rehabilitation processes.
