Role Overview

BayCare is seeking a TOC Clinical Pharmacist who is committed to excellent patient service and helping improve care after discharge.

What the Role Involves

The pharmacist will contribute to reducing readmissions by supporting discharge follow-up and providing clinical care that strengthens medication safety, patient understanding of medicines and disease conditions, and continuity with healthcare providers after leaving the hospital.

• Location: Dunedin, Florida
• Employment status: Full time, exempt
• Shift timing: 8:00 AM to 4:30 PM
• Weekend availability: Occasional
• On-call requirement: No

Licensing and Certification

A Florida pharmacist license is required. ASHP pharmacist certification is preferred.

Education

A bachelor's degree is required. PharmD is preferred.

Experience

PGY-1 residency is preferred, and PGY-2 residency is also preferred.

Compliance Note

This role requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, administered by the Agency for Health Care Administration (AHCA).
