Job Overview
The Medical Coder role involves converting information from patient records such as physician notes, lab reports, procedures, and diagnoses into standardized medical codes. These codes are used to support accurate medical records, billing, reporting, reimbursement, and compliance.
Role Details
Reporting To: Team Leader
Experience: 1 to 5 years
Qualification: Any degree with a life science background
Location: Bangalore or Coimbatore
Shift: 9:00 AM to 6:00 PM, day shift
Work Mode: Work from office
Employment Type: Full-time
Responsibilities
- Code and abstract patient encounters, including diagnosis details, procedures, notable reportable items, and complications.
- Study reimbursement requirements and investigate data needed for billing accuracy.
- Review medical records to spot missing or incomplete documentation.
- Check that diagnoses, procedures, and treatment outcomes are properly supported by the record.
- Identify the relevant diagnostic and procedural information from clinical documents.
- Audit medical documentation and coded data to confirm that services are supported for reimbursement and reporting.
- Apply coding guidelines to support reimbursement, research, and regulatory compliance.
- Act as a reference point for other coding team members and provide subject-matter guidance.
- Support training, mentoring, and onboarding of staff when required.
- Take on special assignments as requested by the team lead or supervisor.
Requirements
- Strong working knowledge of coding conventions and medical coding practices.
- Ability to advise healthcare providers on coding-related matters.
- Capability to identify mismatches, billing concerns, and possible quality-of-care issues.
- Skill in researching issues, suggesting corrective actions, and helping prevent future coding errors.
- Ability to recognize reportable elements, complications, and other relevant procedures.
- Willingness to assist in training and mentoring junior team members.
- Must be a certified coder.
- Experience in orthopedics, surgery, E&M, and emergency department specialties will be an added advantage.
