Position Overview
The Education Continuity Coordinator (ECC) serves a crucial role ensuring continuous access to educational rights and services for adolescents and young adults enrolled in Sandstone Care's Partial Hospitalization Program (PHP). Operating under both behavioral health regulations and state educational authorities, the ECC acts as the key liaison connecting the treatment program with students' home schools, Local Education Agencies (LEAs), post-secondary institutions, and relevant education officials within designated states.
Work Setting and Schedule
This full-time position is based in outpatient treatment centers for teens and young adults and follows a Monday through Friday schedule with occasional evenings required. The role is hybrid between Colorado and Maryland locations, primarily onsite.
Key Duties
- Conduct thorough educational assessments for new clients upon admission.
- Analyze and interpret Individualized Education Plans (IEPs) and Section 504 Plans to develop, implement, and track continued learning objectives.
- Deliver academic and vocational support including administering assessments, setting goals and measurable outcomes, and coordinating services with external schools, BOCES, families, and recreational programs.
- Ensure timely and accurate documentation of all educational services provided.
- Assist clients with completing homework and promote their academic engagement.
- Maintain positive public relations with schools, families, referral sources, and community agencies.
- Effectively communicate ideas and information clearly and persuasively to diverse audiences.
- Exercise sound judgment and emotional resilience when making decisions under stress or time constraints.
- Participate in Individualized Education Plan (IEP) meetings as needed.
- Support and coordinate a structured daily study period for clients.
- Collaborate with Site Director to monitor clients' anticipated treatment durations.
- Maintain detailed communication logs with schools for each active client.
- Prepare comprehensive Academic Transition Plans prior to client discharge.
- Engage in monthly quality assurance reviews to uphold service standards.
Qualifications
- Bachelor's degree required; preference for degrees in Education or Behavioral Health Sciences such as clinical social work, psychology, or marriage and family therapy.
- Possession of or eligibility for a teaching license within the working state.
- Familiarity with the Individuals with Disabilities Education Act (IDEA).
- Experience mentoring adolescents and providing academic, vocational, and life skills support.
- At least three years of relevant experience working with clients aged 13 to 18 preferred.
- Strong written and verbal communication proficiency.
Compensation and Benefits
Pay rates range from $25 to $30 per hour depending on experience and licensure status.
