Intake Coordinator Registered Nurse (RN) - Full Time Evening Shift

Job Overview

Join our team as a full-time Intake Coordinator Registered Nurse working the evening shift at Fremont Hospital. The shift runs from 3:00 PM to 11:30 PM. This role is integral to the Assessment & Referral Team, performing initial medical screenings of incoming behavioral health patients and collaborating with medical staff to direct patients to their appropriate care levels.

Location and Accessibility

The hospital is conveniently accessible via public transport including AC Transit, BART, and ACE Train.

Responsibilities

• Conduct Initial Medical Screening Assessments for new patients.
• Work closely with medical professionals to determine suitable patient care pathways.
• Participate actively as a core member of the Assessment & Referral Team.

Qualifications and Experience

• An Associate's degree in nursing is mandatory; Bachelor's degree preferred.
• Candidates with advanced degrees such as Master's in social work, psychology, related mental health fields, or nursing doctorates are favored.
• At least one year of direct clinical experience in psychiatric or mental health settings is required.
• Experience must include patient assessment, engagement with families, treatment planning, and liaison with external review bodies.
• Active and valid RN license issued by the California Board of Registered Nursing is necessary.
• Current CPR and AED certifications are required and must be submitted to Human Resources upon hiring.
• Completion of Fremont Hospital’s non-violent Crisis Prevention Intervention training within 90 days of employment is mandatory.
• Flexibility to work overtime and varied hours if needed.
