About Transitional Therapeutic Services

Role Overview

The Qualified Professional (QP) plays a crucial role in delivering person-centered mental health care to individuals and families. Responsibilities include crisis management, life skills development, case management, and participation in emergency responses. Reporting to the team lead, the QP works collaboratively within an interdisciplinary team to ensure thorough and effective care.

Key Responsibilities

  • Create, implement, monitor, and refine Person-Centered Plans (PCP) in partnership with healthcare team members.
  • Act as a liaison facilitating communication among clients, families, educational institutions, community organizations, and local agencies.
  • Provide support interventions addressing behavioral issues, mental health conditions, and substance dependencies.
  • Help clients build vital life skills such as daily living, social interaction, behavior regulation, and anger control.
  • Participate in a 24/7 on-call system to deliver crisis intervention and immediate support when necessary.
  • Organize and facilitate service coordination, referrals, and resource connections for clients.
  • Collaborate closely with clinical staff to maintain standardized processes for PCP deployment.
  • Oversee and evaluate service utilization ensuring effectiveness, appropriateness, and regulatory adherence.
  • Lead monthly Child and Family Team meetings to promote cooperative engagement and monitor progress.

Required Qualifications and Experience

  • Bachelor's degree in a Non-Human Services field plus a minimum of 4 years full-time experience with the target population.
  • Or, Bachelor's degree in Human Services with at least 2 years of full-time relevant experience.
  • Or, Master's degree or higher with at least 1 year of full-time relevant experience.

Relevant majors include Social Work, Psychology, Sociology, and related Human Services disciplines. The primary populations served are children and adolescents with mental health or substance use challenges.

Work Schedule

Flexible working hours are essential, including availability during evenings, weekends, and on-call periods to meet client and stakeholder needs.
