What you will do

  • Partner with the Enhanced Maternal and Child Health teams to coordinate extra support for referred families, helping them strengthen parenting capacity in line with relevant MCH and EMCH guidelines.
  • Work with Maternal and Child Health Nurses to support the ongoing needs of families referred into the service.
  • Take part in intake, case management, team meetings, and clinical supervision on a regular basis.
  • Provide intensive individual support to families in different settings, including home visits and group environments, by identifying strengths and setting realistic goals.
  • Support families directly and facilitate parenting and family-focused groups.
  • Help improve child health, wellbeing, safety, learning, and development by connecting families with suitable services such as Supported Playgroup, Early Start Kinder, Access to Early Learning, and Preschool Field Officers.
  • Prepare and deliver up-to-date, evidence-based information and education sessions for groups in line with Maternal and Child Health and Department of Health requirements and funding.
  • Record all family consultations accurately and in a timely way on the CDIS database, following Maternal and Child Health documentation standards.
  • Assist with reports and related preparation within required deadlines.

Skills and experience

  • Strong time management skills, with the ability to prioritise tasks and organise workloads within Enhanced Maternal and Child Health services.
  • Ability to support families toward agreed outcomes through case management, a strengths-based approach, group facilitation, and report contribution.
  • Capability to work both independently and as part of a team, with a high level of initiative.
  • Sound judgement in identifying family needs and expectations and making appropriate decisions with support from colleagues.
  • Collaborative approach that contributes to team cohesion and overall effectiveness.
