Role Overview
This position demands strong critical thinking abilities, clear communication, and sound decision-making to foster constructive relationships and guide others effectively. The Case Manager will educate patients, families, and healthcare teams about care pathways, service use, appropriate care levels, and facilitate safe patient transitions.
Qualifications
- At least one year of experience in hospital-based Case Management or a minimum of five years in the healthcare sector.
- Graduated from an accredited Registered/Professional Nursing Program.
- Must obtain Basic Life Support (BLS) Healthcare Provider certification upon hiring.
